This short online form is the quickest and simplest way for the majority of our patients to access or clinical team for assistance.
Our practice phone lines remain open for anyone with accessibility needs who might find difficulty completing this form.
- Please do not use this online form for urgent or emergency requests.
- It is available for you to submit Monday to Friday 09.00-16.00.
- Please contact 111 during out of hours or 999 if this is an emergency.
How do I use Patient Triage?
- Open the link from our practice website (There’s no need to download an app or create an account!)
- Select either admin or medical request
- Confirm this is a non urgent request
- Write the request to the practice, giving as much information as possible
- Tell the practice how you want to be contacted
- Enter your details
- Press submit!
